A recent US study of miners provides some of the strongest evidence of an association between exposure to diesel-engine exhaust and lung cancer since there were few potential confounding exposures. Lipsett and Campleman (1999) in a meta-analysis of 12 studies (20 risk estimates) that had adjusted for smoking obtained a pooled smoking-adjusted RR of 1.47 (95% CI 1.29–1.67). Exposure to diesel engine emissions occurs in many occupational settings, and it is often difficult to evaluate separately from other combustion fumes, notably gasoline engine emissions.
